Job Title: Care Navigator

At Carolina Complete Health, we're transforming the health of our communities, one person at a time. As a Care Navigator, you'll play a vital role in developing and coordinating care management activities to provide quality, cost-effective healthcare outcomes for our 28 million members.

Key Responsibilities:
  • Develop personalized care plans and educate members and their families/caregivers on services and benefit options to improve healthcare access and receive appropriate high-quality care.
  • Evaluate member needs, barriers to care, and resources available, and recommend and facilitate plans for the best outcome.
  • Provide psychosocial and resource support to members/caregivers and care managers to access local resources or services.
  • Coordinate between members and/or family/caregivers and the care provider team to ensure identified care or services are accessible in a timely manner.
  • Monitor progress towards care plans/service plans goals and/or member status or change in condition, and collaborate with healthcare providers for care plan/service plan revision or address identified member needs.
Requirements:
  • Bachelor's degree and 2 – 4 years of related experience.
  • Current state's clinical license preferred.
