Job DescriptionCare Coordinator - Physical Therapy Specialist Job Summary

The Care Coordinator - Physical Therapy Specialist is a qualified professional who directs, supervises, evaluates, and provides physical therapy services to patients as prescribed by the attending physician.

Responsibilities
  • Comprehensive evaluation of the patient's level of function and participation in the development of the total patient care plan in conjunction with RN (if applicable), patient, family, and physician.
  • Participation in case conferences/ team huddles. Ensure proper documentation.
  • As needed by the agency, supervision of Home Health Aides and when such personnel is participating in the patient's therapy regime.
  • Instruction of patient, family, and other agency health care personnel in the patient's treatment regime when indicated
  • Assists patients to reach a level of function to enable them to remain safely in their home environment while reaching national standards for outcomes and maintaining fiscal responsibility
  • Looking for someone to perform in a PART-TIME or PER-DIEM capacity
Qualifications
  • Graduate with at least a baccalaureate degree from an accredited school of Physical Therapy approved by the American Medical Association Council of Medical Education in collaboration with the American Physical Therapy Association.
  • Current license to practice as a Physical Therapist
  • At least two (2) years of clinical experience.
  • Knowledge of physical therapy methods with particular skills in casework in the home setting.
