Job Summary:

OakBend Medical Center is seeking a highly skilled and compassionate Healthcare Navigator to join our team. As a key member of our patient care team, you will play a vital role in connecting patients with relevant community resources, enhancing their health and well-being, and reducing readmissions.

Responsibilities:

  • Collaborate with patients, family members, and healthcare providers to develop personalized care plans.
  • Identify and connect patients with community resources, such as social services, transportation, and housing assistance.
  • Provide education and support to patients and their families on health-related topics, such as disease management and wellness.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
  • Participate in quality improvement initiatives to enhance patient outcomes and satisfaction.

Qualifications:

  • Education: High School Graduate or GED Equivalent; relevant experience in lieu of High School Diploma/GED education will be considered.
  • Experience: 3 years previous customer service experience.
  • Licenses/Certifications: None
  • Skills: Previous experience in a customer support role, strong phone and verbal communication skills, active listening, customer focus, adaptability, ability to multi-task, prioritize, and manage time effectively.
