Job Description

We are seeking a skilled Speech Language Pathologist to join our team at Alternate Solutions Health Network LLC. As a Home Health Speech Therapist, you will play a vital role in providing high-quality care to our patients in the comfort of their own homes.

Key Responsibilities:
  • Develop and Implement Treatment Plans: Create individualized treatment plans for patients with speech and language disorders, and implement them in a home-based setting.
  • Provide Skilled Therapy Services: Deliver skilled speech therapy services to patients, including assessment, diagnosis, and treatment of speech and language disorders.
  • Collaborate with Interdisciplinary Team: Work closely with our interdisciplinary team, including physicians, nurses, and other healthcare professionals, to ensure comprehensive care for our patients.
  • Communicate with Patients and Families: Educate patients and their families on speech and language disorders, and provide guidance on how to manage and improve communication skills.
  • Maintain Accurate Records: Document patient progress, treatment plans, and other relevant information in accordance with agency policies and regulatory requirements.
Requirements:
  • Current License: Must be a licensed Speech Therapist in the state of employment.
  • Experience: Minimum one year of experience as a Speech Therapist in an acute care setting, with home care experience preferred.
  • Valid Driver's License: Must have a valid driver's license and auto insurance in your name as a driver.
  • Physical Demands: Capable of all physical demands of the job.
