Job Title: Triage RN

Gulfside Healthcare is seeking a skilled and compassionate Triage RN to join our team. As a Triage RN, you will play a critical role in identifying, prioritizing, and coordinating patient/family needs to provide preventative and supportive care to terminally ill patients and their families.

Key Responsibilities:
  • Attend to patient/family complaints and concerns, taking a proactive approach to identifying and resolving potential issues.
  • Educate the community regarding hospice care, philosophy, and concept through individual consultation with persons in the community.
  • Perform other duties as requested by the Clinical Care Manager, including providing assessment of the physical, psychological, social, and spiritual needs of the patient/family unit and initiating appropriate intervention.
  • Determine the health status of the patient based on hospice criteria and identifies patient/family issues, providing follow-up for information related to patient care issues and continuity of care to the appropriate CCM/RN Case Manager.
  • Communicate pertinent information to the CCM and necessary disciplines to ensure continuity of care, coordinating and identifying services needed by patient/family unit after hours regarding continuous care, transfers, staffing, physician's orders, referrals, pharmacy, and durable medical equipment.
  • Demonstrate knowledge of Gulfside Hospice policies and procedures and pertinent federal and state regulations, considering financial implications in rendering cost-effective nursing care.
  • Ensure professional standards of nursing practice are maintained, performing other duties as requested by the Clinical Care Manager.
Requirements:
  • Currently licensed in the State of Florida as a registered nurse.
  • Minimum of two (2) years nursing experience, with hospice, oncology, or home health nursing experience preferable.
  • Minimum of one (1) year experience in an acute care unit is desirable, as well as minimum of one (1) year experience in caring for terminally ill patients.
  • Should have training in palliative care and symptom management.
