About the Role

As a Nurse Advocate for Patient Care, you will play a vital role in ensuring high-quality patient outcomes by assessing patient needs, developing individualized care plans, and collaborating with interdisciplinary teams. Your responsibilities will include:

  • Conducting thorough assessments of patient symptoms and conditions.
  • Developing and implementing patient care plans in consultation with healthcare professionals.
  • Providing patient education and support on healthcare information and resources.
  • Supervising and evaluating home healthcare staff as needed.
  • Participating in on-call rotations and offering backup support as required.

Requirements

  • Current RN licensure in the state of operation.
  • Prior experience in hospice care or a related field is highly valued.
  • Strong understanding of nursing principles and patient-centered care practices.
