Homecare LPN Position

Centers Plan for Healthy Living is seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our homecare team. As an LPN, you will provide high-quality nursing care to clients in their homes, promoting their health and well-being.
Responsibilities:
Provides skilled nursing care to clients, including treatment, medication administration, and treatment as directed by the Registered Nurse and the Physician's Plan of care.
Promotes and maintains client health through teaching and appropriate rehabilitative measures under the direction of the Registered Nurse.
Helps to achieve and maintain continuity of client care by completing exchanging information with other health professionals regarding patient care issues.
Completes clinical progress notes and follows client care plan as developed by the Registered Nurse and according to Agency policies and procedures.
Documents and reprints all changes in physical and behavioral status, all accidents, incidents, events, and injuries.
Requirements:
Minimum 2 years of experience as a Licensed Practical Nurse.
Active NYS LPN license.
