Medical Social Worker

CenterWell Home Health is seeking a skilled Medical Social Worker to join our team. As a Medical Social Worker, you will play a vital role in providing comprehensive care to patients in their homes.
Key Responsibilities:
- Evaluate and develop personalized plans of care to address patients' emotional and social needs.
- Assess patients' social and emotional states, identifying areas for improvement and developing strategies for resolution.
- Collaborate with healthcare teams to ensure seamless care coordination and patient outcomes.
- Supervise and educate Social Work Assistants to ensure high-quality patient care.
Requirements:
- Masters or doctoral degree from a school of social work accredited by the Council on Social Work Education.
- Social Worker licensure in the state of practice (if required by state law or regulation).
- Valid driver's license, auto insurance, and reliable transportation.
- Proof of current CPR certification.
- Minimum of one year of experience as a social worker in a healthcare setting, home health, and/or hospice.
