Public Health Nurse

We are seeking a highly skilled and dedicated Public Health Nurse to join our team at McLeod County. As a Public Health Nurse, you will play a vital role in promoting health and preventing disease in our community.
Key Responsibilities- Conduct comprehensive individual and family assessments to identify health needs and develop care plans.
- Provide care coordination, case management, and referral services to vulnerable populations and high-risk groups.
- Develop and implement health education programs and activities to promote healthy behaviors and prevent disease.
- Collaborate with healthcare providers and community organizations to ensure seamless care coordination and service delivery.
- Participate in public health emergency response efforts and provide support to community members in need.
- Current licensure as a registered nurse in the State of Minnesota.
- Associate degree in nursing and two years of experience or equivalent.
- Valid Minnesota driver's license.
- Current Health Care CPR certification within the past 12 months.
- Experience in public health nursing.
- Experience working in community settings.
- Bachelor's degree in nursing from an accredited college.
- Public Health Nurse certificate.
- Ability to speak and interpret Spanish.
This position requires the ability to work in a variety of settings, including offices, community clinics, schools, and client homes. The employee must be able to relate to members of the public who may exhibit challenging behaviors and communicate effectively in person and over the phone.
Physical RequirementsThe employee must be able to sit or stand for extended periods, hear and speak effectively, and have visual acuity to read computer screens and medical instrument displays. The employee must also be able to apply physical coordination to safely drive an automobile and enter small spaces in motor vehicles.
