Position Summary
The RN Case Manager is responsible for integrating healthcare services for clients from pre-admission through post-discharge. This role involves coordinating and sequencing client care to enhance patient flow and improve interdisciplinary communication, facilitating early interventions. The RN will evaluate outcomes based on established clinical, financial, and utilization data.
This position operates in both structured and unstructured healthcare environments, which may lack established policies and procedures. The RN will exercise independent nursing judgment in the integration of care.
The role requires knowledge of age-specific care methodologies, including protocols and clinical pathways, to address the healthcare needs of various patient populations, including neonatal, pediatric, adolescent, adult, and geriatric clients. The RN must effectively communicate with patients, families, and the healthcare team, coordinating and negotiating care as necessary. Responsibilities also include monitoring and evaluating patient responses using structured care methodologies while maintaining fiscal responsibility and controlling variances.Critical thinking, independence, and assertiveness are essential when interacting with patients and colleagues. The RN must document care clearly, solve problems effectively, and work autonomously. The ability to incorporate teaching principles and adapt educational content to different age groups is crucial. Organizational skills and flexibility are necessary to prioritize tasks effectively. Knowledge of reimbursement processes for provided services is also required.
Work primarily involves day hours, with scheduled weekends and occasional evenings. The RN may encounter communicable diseases, hazardous materials, and various environmental stressors while performing assigned duties.
Qualifications
A Bachelor’s degree in nursing is preferred.
Graduates from nationally accredited nursing programs are highly regarded, including those accredited by the Commission on Collegiate Nursing Education (CCNE), Accreditation Commission for Education in Nursing (ACEN), and National League for Nursing Commission for Nursing Education Accreditation (NLN CNEA).
A minimum of one to two years of professional nursing experience in a clinical setting is preferred, with in-depth knowledge of the specialty area.Must hold an unencumbered RN license from the State Board of Nursing where practice occurs, or possess multistate licensure if in a Nurse Licensure Compact (NLC) state.
Required competencies and certifications must be obtained and maintained as per departmental standards. A valid driver’s license may be necessary depending on the location.Benefits
