Position Summary
This role is essential in coordinating comprehensive health care for clients, ensuring a seamless transition from pre-admission through to post-discharge. This is achieved by effectively managing and sequencing the client’s care, which enhances patient flow and fosters interdisciplinary communication for timely interventions. The position also involves evaluating outcomes based on established clinical, financial, and utilization metrics.

Operates in both structured and unstructured healthcare environments, which may lack established policies and procedures. Employs independent nursing judgment to integrate health care effectively.

Possesses knowledge of and applies age-specific care methodologies, including protocols and clinical pathways, tailored to the health care needs of various patient demographics, including neonatal, pediatric, adolescent, adult, and geriatric populations. Exhibits strong communication skills to engage with patients, family members, and the healthcare team. Coordinates, facilitates, and negotiates care plans while monitoring and evaluating patient responses using structured care methodologies. Maintains fiscal responsibility by controlling variances and avoiding duplication of services.

Demonstrates independence, assertiveness, and critical thinking in interactions with patients and colleagues. Capable of clear documentation and problem-solving. Self-motivated and able to work with minimal supervision. Adapts teaching methods to suit different age groups and facilitates group sessions and project implementations as needed. Must prioritize effectively, exhibit flexibility, and possess strong organizational skills. Knowledgeable about reimbursement processes for provided services.

Availability for primarily day shifts, scheduled weekends, and occasional evenings is required. May encounter communicable diseases, hazardous materials, and physical risks associated with job duties. Subject to various sensory and environmental stressors.

Qualifications
Bachelor’s degree in nursing is preferred.


Graduation from a nationally accredited nursing program is preferred, including but not limited to, Commission on Collegiate Nursing Education (CCNE), Accreditation Commission for Education in Nursing (ACEN), and National League for Nursing Commission for Nursing Education Accreditation (NLN CNEA).

A minimum of one to two years of professional nursing experience in a clinical setting is preferred, with in-depth knowledge of the specialty area.


Must hold an unencumbered RN license with the State Board of Nursing in the practice area and/or possess multistate licensure if applicable.

Must obtain and maintain required department-specific competencies and certifications. A valid driver’s license may be necessary depending on the location.
