Job Summary

As a RN Case Manager with the Good Samaritan Society, you will play a vital role in integrating healthcare services for clients from pre-admission to post-discharge. This involves coordinating and sequencing client care to enhance patient/resident flow and promote interdisciplinary communication.

Key Responsibilities
  • Utilize independent nursing judgment to integrate healthcare services
  • Develop and implement structured care methodologies to meet the unique needs of clients
  • Collaborate with healthcare teams to coordinate care and promote early intervention
  • Monitor and evaluate patient/resident responses to care, identifying areas for improvement
  • Fiscally responsible, controlling variances and duplication in care
Requirements
  • Bachelor's degree in nursing preferred
  • Minimum of one to two years of professional nursing experience in a clinic care setting
  • Currently holds an unencumbered RN license with the State Board of Nursing
  • Obtains and maintains required department-specific competencies and certifications
