Job Title: Licensed Practical Nurse (LPN/LVN)

Aveanna Healthcare LLC is seeking a dedicated Licensed Practical Nurse (LPN/LVN) to provide high-quality patient care in a home health setting.

Job Summary:

The Licensed Practical Nurse (LPN/LVN) will be responsible for delivering and documenting skilled nursing care to patients in their homes, under the supervision of a Registered Nurse. The ideal candidate will have excellent communication skills, a strong attention to detail, and the ability to work independently.

Responsibilities:
  • Deliver and document skilled nursing care to patients in their homes, under the supervision of a Registered Nurse.
  • Conduct thorough assessments of patients' physical and emotional needs, and develop and implement individualized care plans.
  • Provide education and support to patients and their families on disease management, self-care techniques, and prevention strategies.
  • Collaborate with the healthcare team to ensure seamless care coordination and communication.
  • Maintain accurate and timely documentation of patient care, including progress notes, medication administration records, and other relevant information.
Requirements:
  • Graduate of an accredited school of nursing.
  • Current, unrestricted state license as a Licensed Nurse in the state of practice.
  • Current CPR certification.
  • Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care and policies and procedures.
Preferred Qualifications:
  • Six (6) months of recent experience as a Licensed Nurse in a clinical care setting.
  • Home health experience.
Physical Requirements:
  • Must be able to speak, write, read, and understand English.
  • Must be able to travel.
  • Must be able to lift 50 pounds.
  • Must be able to sufficiently reposition patients and move equipment without assistance.
  • Prolonged walking, standing, bending, kneeling, reaching, twisting.
  • Must be able to sit and climb stairs.
  • Must have visual and hearing acuity.
  • Must have a strong sense of smell and touch.
  • Must be able to sufficiently reposition patients and move equipment without assistance.
  • Must be able to appropriately respond physically and mentally to emergency situations in the home or during transport.
Environment:
  • Must be able to function in a wide variety of environments which may involve exposure to allergens and other various conditions.
  • Possible exposure to blood, bodily fluids, and infectious diseases.
