Job Title: Licensed Practical Nurse

At Yale New Haven Health, we are seeking a highly skilled and compassionate Licensed Practical Nurse to join our team. As a key member of our healthcare team, you will play a vital role in providing high-quality patient care and contributing to the overall success of our organization.

Responsibilities:
  • Provide direct patient care, including administering medications, performing treatments, and documenting patient information.
  • Supervise and direct nursing assistants, ensuring that they are providing high-quality care and adhering to facility policies and procedures.
  • Conduct resident rounds, identify potential problems, and take corrective action as necessary.
  • Prepare and assign nursing assistants' assignments and duties, ensuring that they are adequately trained and equipped to provide quality care.
  • Perform data collection, develop comprehensive care plans, and participate in resident care conferences.
  • Document patient information, including medications administered, treatments provided, and changes in patient condition.
  • Counsel nursing assistants and recommend disciplinary action when necessary.
  • Participate in the employee evaluation process and provide information on the clinical performance of certified nursing assistants.
  • Perform other clinical duties, including checking emergency equipment, identifying safety hazards, and notifying physicians of changes in patient condition.
Qualifications:
  • Graduated from an approved Practical Nurses Education Program.
  • At least one year of work experience in rehabilitative, psychiatric, or geriatric nursing preferred.
  • A Licensed Practical Nurse licensed in the State of Connecticut.
  • Excellent communication and interpersonal skills.
