As a skilled Home Health Registered Nurse, you will play a vital role in delivering high-quality, patient-centered care to individuals in the comfort of their own homes. Your expertise will enable patients to recover, rehabilitate, and regain their independence, empowering them to live healthier and happier lives.

Key Responsibilities:
  • Provide comprehensive skilled nursing visits, including admission, case management, and follow-up care, to ensure patients receive the best possible outcomes.
  • Collaborate with physicians to develop and implement individualized care plans, incorporating the latest evidence-based practices and patient preferences.
  • Conduct thorough assessments, identify areas for improvement, and develop strategies to address patient needs, working closely with the care team to ensure seamless coordination.
  • Coordinate with other healthcare professionals, such as physical therapists, occupational therapists, and home health aides, to provide a holistic approach to patient care.
  • Communicate effectively with patients, families, and healthcare providers to ensure informed decision-making and optimal care.
  • Document patient progress, outcomes, and care plans accurately and thoroughly, adhering to regulatory requirements and CenterWell Home Health standards.
Requirements:
  • Diploma, Associate, or Bachelor's degree in Nursing
  • Minimum one year of nursing experience, preferably in med-surg, ICU, ER, or acute care settings
  • Strong organizational and communication skills, with the ability to work independently and as part of a team
  • Current and unrestricted Registered Nurse licensure, as well as CPR certification
  • Valid driver's license, auto insurance, and reliable transportation
