Key Responsibilities:- Provide comprehensive skilled nursing visits, including admission, case management, and follow-up care, to ensure patients receive the best possible outcomes.
- Collaborate with physicians to develop and implement individualized care plans, incorporating the latest evidence-based practices and patient preferences.
- Conduct thorough assessments, identify areas for improvement, and develop strategies to address patient needs, working closely with the care team to ensure seamless coordination.
- Coordinate with other healthcare professionals, such as physical therapists, occupational therapists, and home health aides, to provide a holistic approach to patient care.
- Communicate effectively with patients, families, and healthcare providers to ensure informed decision-making and optimal care.
- Document patient progress, outcomes, and care plans accurately and thoroughly, adhering to regulatory requirements and CenterWell Home Health standards.
- Diploma, Associate, or Bachelor's degree in Nursing
- Minimum one year of nursing experience, preferably in med-surg, ICU, ER, or acute care settings
- Strong organizational and communication skills, with the ability to work independently and as part of a team
- Current and unrestricted Registered Nurse licensure, as well as CPR certification
- Valid driver's license, auto insurance, and reliable transportation
