As a skilled Home Health Registered Nurse, you will play a vital role in delivering high-quality, patient-centered care to individuals in the comfort of their own homes. Your expertise will enable patients to recover, rehabilitate, and regain their independence, empowering them to live healthier and happier lives.

Key Responsibilities:
  • Provide comprehensive, skilled nursing visits to home health patients, ensuring their needs are met and their health is optimized.
  • Collaborate with the clinical team to develop and implement personalized care plans, addressing the unique needs and goals of each patient.
  • Conduct thorough assessments, identify areas for improvement, and implement evidence-based interventions to enhance patient outcomes.
  • Communicate effectively with patients, families, and healthcare providers to ensure seamless care coordination and optimal patient satisfaction.
  • Monitor patient progress, adjust care plans as needed, and provide education and support to patients and their caregivers.
  • Supervise and mentor home health aides and other healthcare professionals to ensure high-quality care delivery.
  • Document patient information accurately and thoroughly, maintaining confidentiality and adhering to regulatory requirements.
Requirements:
  • Diploma, Associate, or Bachelor's degree in Nursing
  • Minimum one year of nursing experience, preferably in home health or a related setting
  • Strong med-surg, ICU, ER, or acute care experience
  • Current and unrestricted Registered Nurse licensure
  • Current CPR certification
  • Excellent communication and organizational skills
  • Valid driver's license, auto insurance, and reliable transportation
