Social Worker Level II Position

Kaiser Permanente is seeking a skilled Social Worker to join their team in San Francisco, California. As a Medical Social Worker II, you will play a vital role in assisting patients and families cope with the social and emotional issues related to their illness.
Job Summary:The primary responsibility of this position is to deliver age-appropriate social work care to patients and their caregivers in accordance with agency policy and procedure. You will work under the general direction of the Social Work Manager/LCSW to provide assessments, coordination, treatment planning, and information and referral to community resources.
Key Responsibilities:- Provide psychodynamic interventions, crisis intervention, grief/bereavement counseling, problem solving, stress reduction, and developing healthy coping strategies in individual/family/group settings.
- Develop and implement individual Plans of Treatment to assist patients and families to cope and/or restore social, emotional, financial, and environmental factors affected by illness.
- Complete psychosocial assessments and partner with patients to identify needs and develop and implement individual treatment plans.
- Discuss options for care proactively, including Kaiser resources and external community/government resources, to assist patients and families in developing short and long-term care plans.
- Work with other disciplines in assessing, planning, and providing services for patients utilizing biopsychosocial information.
- Assist patients in advocating for themselves to receive appropriate services within Kaiser and in the community.
- Assist patients and families with care planning and discharge plans.
- Maintain documentation, records, and data collections.
- Responsible for completion of required documents in a complete and timely manner.
- Masters in Social Work accredited by the Council of Social Work Education.
- 2-4 years of social work experience within the last five years.
- Demonstrated ability to work on a multidisciplinary team.
- Solid psychosocial assessment skills.
- Knowledge of chronic and acute disease and how it impacts patient and family functioning.
- Excellent oral/telephone communication skills and written documentation.
- Computer-literate and experienced in automated clinical information systems.
- Ability to effectively and efficiently handle demanding workload involving multiple tasks.
