Per Diem Medical Social Worker II

We are seeking a highly skilled and compassionate Per Diem Medical Social Worker II to join our team at Sutter Health. As a key member of our interdisciplinary team, you will provide exceptional patient care and support to patients and their families in a variety of settings.
Responsibilities:- Conduct biopsychosocial assessments and provide crisis intervention, short-term counseling, and advocacy to patients and their families.
- Develop and implement plans for transitions of care, ensuring seamless coordination with other healthcare providers.
- Provide education and support to patients and families, addressing their unique needs and concerns.
- Collaborate with the multidisciplinary treatment team to ensure comprehensive care and effective discharge planning.
- Participate in committees and contribute to the development of policies and procedures that promote high-quality patient care.
- Master's degree in Social Work or a related field.
- Current MSW-Medical Social Worker certification.
- Excellent clinical skills, including biopsychosocial assessment and intervention.
- Strong collaboration and communication skills, with the ability to work effectively in a fast-paced environment.
- Knowledge of local county/state/federal resources for at-risk populations.
- Ability to work variable shifts, including weekends and rotating shifts.
- A competitive hourly rate of $52.01 to $70.20.
