LPN Program Manager

We are seeking a highly skilled Licensed Practical Nurse (LPN) to join our team as a Medical Liaison at Harc Inc. The ideal candidate will have excellent communication skills, be able to work independently, and demonstrate a strong commitment to providing high-quality care to individuals with developmental disabilities.
About the Position:
- Work closely with the Program RN and DON to ensure accurate transcription of doctor's orders, communicate with pharmacies, and monitor medication errors.
- Assist with completing monthly Med Room Closet Audits, destroying expired medications, and maintaining quarterly med audits and nursing reviews.
- Develop and implement positive educational approaches and reinforcement programs to support individual strengths and adaptive limitations.
Responsibilities:
- Monitor the overall health of supported individuals, including signs and symptoms of chronic and acute medical conditions.
- Communicate current medical information to the Program RN, DON, medical providers, staff, and families.
- Review monthly kardexes, make necessary changes, and assist with maintaining quarterly med audits and nursing reviews.
Benefits: Competitive salary range: $65,000 - $85,000 per year, depending on experience, plus benefits package that includes comprehensive health insurance, retirement plan, and paid time off.
