Job Summary

MedStar Health is seeking a skilled Social Worker to join our Home Care team. As a Social Worker Per Visit, you will provide essential social work services to patients and clients in Montgomery County.

Key Responsibilities
  • Provide counseling and linkage with community services to patients and clients as needed.
  • Perform assessments and assist in resolving social problems that may impede patient treatment, rehabilitation, and medical compliance.
  • Collaborate with healthcare team members to understand psychosocial and emotional factors related to patient health problems.
  • Develop and implement plans of social care based on patient needs.
  • Communicate effectively with patients, families, and healthcare team members.
Requirements
  • Master's degree in social work from a Council on Social Work Education-accredited program.
  • 1-2 years of social work experience in a healthcare setting.
  • Licensed Graduate Social Worker (LGSW), Licensed Certified Social Worker (LCSW), or equivalent.
  • CPR certification.
  • Valid State Driver's License.
