We are seeking a skilled and compassionate Social Worker to join our team at Johns Hopkins Care at Home. As a Social Worker, you will play a vital role in providing high-quality, individualized care to patients and families in their homes and communities.

About the Role

As a Social Worker at Johns Hopkins Care at Home, you will work collaboratively with our interdisciplinary team to integrate social work services into the plan of care. Your primary responsibilities will include:

  • Providing direct social work services to patients in their homes
  • Working with the interdisciplinary team to develop and implement individualized care plans
  • Conducting assessments and evaluations to identify patient needs and develop strategies for care
  • Collaborating with patients, families, and healthcare providers to ensure seamless care transitions
Requirements

To be successful in this role, you will need:

  • A Master's Degree in Social Work from an accredited School of Social Work
  • Current licensure as a Licensed Clinical Social Worker (LCSW-C) in the state of Maryland
  • Experience working with adult populations, particularly those with chronic or terminal illnesses
  • Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare providers
