Social Worker - LifeBridge Health

At LifeBridge Health, we are seeking a skilled Social Worker to join our team. As a Social Worker, you will play a vital role in providing direct services to patients and families, identifying and resolving problems that impede discharge planning or the patient's use of healthcare resources.

Key Responsibilities:

  • Assess and develop discharge plans for adult and geriatric patients.
  • Collaborate with physicians, nursing, and other multidisciplinary team members to promote care coordination.
  • Provide emotional, social, and financial support to patients and families.
  • Identify and mobilize community resources to meet patient needs.
  • Maintain accurate and timely documentation of patient interventions.

Requirements:

  • Master's Degree in Social Work/Counseling from an accredited institution.
  • Current licensure in the State of Maryland – LMSW/LGPC required; LCSW-C/LCPC preferred.
  • Minimum of 1 year of experience in a healthcare setting; hospital preferred.
