Job Title: Home Health Registered Nurse

Sanzie Healthcare Services, Inc. is seeking a skilled and compassionate Home Health Registered Nurse to join our team. As a Home Health Registered Nurse, you will play a vital role in providing high-quality, patient-centered care to individuals in their homes.

Job Summary:

We are looking for a dedicated and experienced Registered Nurse to provide home health care services to our clients in DeKalb County. The ideal candidate will have a strong background in nursing, excellent communication skills, and the ability to work independently in a fast-paced environment.

Responsibilities:
  • Conduct thorough assessments of patients' health status and develop individualized care plans to meet their unique needs.
  • Provide skilled nursing care, including wound care, medication management, and education to patients and their families.
  • Collaborate with other healthcare professionals to ensure seamless care coordination and communication.
  • Monitor patients' progress, identify potential complications, and make adjustments to care plans as needed.
  • Document all patient interactions, assessments, and care provided in a timely and accurate manner.
Requirements:
  • Current and unencumbered license to practice as a Registered Nurse in Georgia.
  • Minimum of two years of experience in home health care or a related field.
  • Ability to work independently and as part of a team.
  • Excellent communication and interpersonal skills.
  • Valid driver's license and reliable transportation.
