About the Role

We are seeking a highly skilled and compassionate Registered Nurse to join our Home Therapies team as a CAP 1 RN. As a key member of our interdisciplinary team, you will play a vital role in providing exceptional patient care and support to individuals with end-stage kidney disease.

Key Responsibilities
  • Assess and manage patients' response to home dialysis training and treatment therapy, following established protocols and communicating with physicians as needed.
  • Collaborate with the Interdisciplinary Team to develop and implement individualized care plans, ensuring seamless transitions and optimal patient outcomes.
  • Provide education and training to patients and their families on safe and effective dialysis therapy, home care, and medication management.
  • Conduct regular assessments and evaluations to identify and address any barriers to successful home dialysis, working closely with patients and their support networks to develop solutions.
  • Participate in quality improvement initiatives and contribute to the development of policies and procedures to enhance patient care and facility operations.
Requirements
  • Current RN licensure in the state of employment
  • Minimum 1 year of experience as a Registered Nurse, with a strong background in nephrology nursing
  • Home dialysis therapy experience preferred
  • Ability to work effectively in a fast-paced environment, prioritizing multiple tasks and responsibilities
  • Excellent communication and interpersonal skills, with a focus on patient-centered care
