Mobile Diversion Case Manager

We are seeking a highly skilled and compassionate Mobile Diversion Case Manager to join our team at Crossroads Rhode Island. As a key member of our organization, you will play a vital role in providing short-term, solutions-focused crisis intervention and resources to individuals and families experiencing homelessness.
Key Responsibilities- Advocate for the mission and values of Crossroads Rhode Island, promoting a culture of respect and effectiveness in all interactions.
- Deliver services with a focus on safety, respect, and empowerment, ensuring that all clients receive the support and resources they need to achieve stability and self-sufficiency.
- Utilize diversion skills to help individuals and families identify immediate alternate housing arrangements, connecting them with necessary resources and services.
- Assist clients in navigating the benefits and housing systems, including evictions, landlord negotiations, and access to community resources.
- Maintain contact with Coordinated Entry clients on a scheduled basis, prioritizing their needs and status.
- Make referrals to supportive services in the community, supporting clients in navigating the process and achieving their goals.
- Perform administrative duties, including documentation and data entry, to ensure accurate tracking and measurement of services and outcomes.
- Develop and maintain effective collaborative relationships with other community resources, ensuring a comprehensive and coordinated approach to addressing homelessness.
- Assess personal safety in each interaction, taking due diligence to maintain safety and well-being in high-stress situations.
- Ensure confidentiality of service user assessment information, adhering to all relevant legislation and regulations.
- Bachelor's degree in a Human Services field or equivalent experience and education.
- Knowledge of social casework principles, practices, and techniques, with the ability to apply them effectively.
- Ability to work in a fast-paced, outside environment, with excellent communication and interpersonal skills.
- Must be solution-focused, exercising independent judgment and decision-making on an ongoing basis.
- Ability to maintain confidentiality and handle sensitive information with discretion.
- Excellent oral and written communication skills, with proficiency in computer skills and data entry.
