Case Manager

Family Service of Rhode Island is seeking a highly motivated and detail-oriented Case Manager to join our team. As a Case Manager, you will be responsible for providing home-based case management and behavioral support services to clients and their families.
Key Responsibilities:
- Provide home-based case management and behavioral support services to clients and their families.
- Advocate for clients and families to access necessary resources from Human Services and similar departments/agencies.
- Develop and implement individualized service plans to meet the unique needs of clients and families.
- Collaborate with community partners to ensure seamless service delivery.
Requirements:
- Bachelor's degree in a human services field.
- Excellent communication and interpersonal skills.
- Possession of a valid driver's license and reliable transportation.
- Bilingual skills in Spanish.
Physical Requirements:
- Ability to travel to and from clients' residences and community locations.
- Ability to lift up to 20 pounds.
- Ability to communicate effectively with clients and families.
