Job Summary:

Liberty Health is seeking an experienced Hospice Chaplain to join our team. As a Hospice Chaplain, you will provide spiritual support to patients and their families, working collaboratively with our interdisciplinary team to deliver compassionate care.

Key Responsibilities:

  • Provide Spiritual Support: Offer emotional and spiritual support to patients and their families, addressing their unique needs and concerns.
  • Develop and Maintain Resource Group: Establish and maintain a network of clergy and spiritual leaders to support our patients and families.
  • Liaison between Hospice and Community: Serve as a liaison between Liberty Health and the local community, fostering relationships and partnerships to enhance our services.
  • Document Records: Accurately document records of visits and interactions with patients and families.
  • Collaborate with Interdisciplinary Team: Work collaboratively with our interdisciplinary team to deliver comprehensive care and support.
  • Other Job Duties: Perform other duties as assigned to support the delivery of our services.

Requirements:

  • Education: Bachelor's degree from an accredited college or university in religion, human services, theology, philosophy, or a related field. Alternatively, an ordained minister with emphasis in counseling.
  • Experience: Preferably, experience in therapeutic methods of working with individuals, groups, and families in crisis, preferably in counseling and end-of-life care.
  • Skills: Ability to be open, flexible, and ecumenical, with strong communication and interpersonal skills.
