At VNA Health Group, we are seeking a skilled and compassionate Per Diem Home Health Nurse to join our team. As a vital community resource for over 115 years, we provide comprehensive medical care to patients in the comfort of their own homes.

The successful candidate will be responsible for carrying out physical therapy patient care programs under the supervision of a Licensed Physical Therapist. This will include performing therapeutic exercises, mechanical traction, and therapeutic massage, as well as ensuring the proper use and care of equipment such as walkers, crutches, and wheelchairs.

Responsibilities:

  • Carry out physical therapy patient care programs under the supervision of a Licensed Physical Therapist.
  • Perform therapeutic exercises, mechanical traction, and therapeutic massage.
  • Ensure the proper use and care of equipment such as walkers, crutches, and wheelchairs.
  • Modify treatment procedures as indicated by patient response and within the limits specified in the plan of care.
  • Ability to identify situations requiring immediate reports to the Licensed Physical Therapist.
  • Prepare routine reports, administrative procedures, written narratives, schedules, and caseload cards.

Qualifications:

  • Associates Degree approved by the American Physical Therapy Association.
  • Two years' experience as a Physical Therapy Assistant.
  • Licensed in the State of Ohio.
  • CPR certification required.
  • Valid Ohio Driver's License.
  • Experience working as part of a team and working effectively with persons and communities from diverse cultural, social, and ethnic backgrounds.
