Key Responsibilities:
  • Provide patient care across the entire continuum of care, from skilled home health to hospice services.
  • Collaborate with our elite team of healthcare professionals to deliver high-quality care and achieve exceptional patient outcomes.
  • Advocate for patients and their families, ensuring their needs are met and exceeded.
  • Participate in ongoing Continuing Education (CEs) to stay up-to-date on the latest best practices and industry developments.

Requirements:
  • Graduate of an accredited school of professional nursing.
  • Current Certified Licensed Vocational Nurse or Licensed Practical Nurse.
  • 1 year of home care or hospice experience.
  • Able to seek patient feedback, advocate for patients, and understand patient needs and manage patient expectations.
  • Bilingual in English and Spanish preferred.
  • Valid driver's license and insurance.
  • Reliable transportation to perform job tasks.
