Patient Care Assistant

Under the supervision of a licensed nurse, performs a variety of non-professional patient care duties for Health Plan members in a skilled nursing facility (SNF). Responsible for assigned tasks and ensuring the delivery of high-quality patient care.
Key Responsibilities:- Collaborates with the nursing team to develop and implement patient care plans.
- Assists with data collection, identifies changes in patient status, and reports to the licensed nurse.
- Implements care plans, prioritizing tasks with licensed staff.
- Assists with admissions, discharges, and transfers, ensuring patient safety and comfort.
- Reports patient outcomes, unusual responses, or concerns to the licensed nurse.
- Records patient data and nursing care activities in the medical record, adhering to departmental standards.
- Utilizes departmental forms and procedures for documentation.
- Reports incidents, work-related injuries, or safety concerns to the supervisor or designated person.
- Practices infection control principles and standards, ensuring patient, environment, and employee safety.
- Participates in quality improvement activities to enhance patient care.
- Reports patient data and concerns at shift report, informing licensed staff, charge nurse, or supervisor.
- Operates within facility and hospital standards, policies, and procedures.
- High School Diploma or equivalent.
- Completion of a hospital aide training program or equivalent military training.
- Nurse Aide Certification (Hawaii) within 3 months of hire.
- Basic Life Support certification from the American Heart Association within 1 month of hire.
- Minimum one year of experience as a Hospital Aide.
