Job Title: LPN - Home Health

**Job Summary:**

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team in providing high-quality home health care services. As an LPN, you will be responsible for delivering patient-centered care to individuals in their homes, promoting independence, and improving overall health outcomes.

**Responsibilities:**

* Provide skilled nursing care to patients in their homes, including wound care, medication management, and vital sign monitoring
* Develop and implement individualized care plans to meet the unique needs of each patient
* Collaborate with interdisciplinary teams to ensure comprehensive care and coordination
* Educate patients and their families on disease management, medication use, and self-care techniques
* Maintain accurate and timely documentation of patient care and progress

**Requirements:**

* Current LPN licensure in the state of Illinois
* 1+ year of experience in home health care or a related field
* Strong communication and interpersonal skills
* Ability to work independently and as part of a team
* Basic computer skills and familiarity with electronic medical records
