Hospice Liaison Role

Job Summary:

As a Hospice Liaison at LifeBridge Health, you will play a crucial role in ensuring that all services are delivered in accordance with the organization's mission and values. You will serve as a consultant for the hospice program, acting as an advocate for the organization's programs and providing education to healthcare professionals, patients, and families in the community.

Key Responsibilities:

  • Collaborate with healthcare professionals to improve care for patients with life-limiting illnesses
  • Facilitate admission of eligible patients to hospice care in their preferred setting
  • Provide education and support to patients, families, and caregivers
  • Develop and implement strategies to improve patient outcomes and satisfaction
  • Work closely with the hospice team to ensure seamless care coordination

Requirements:

  • Associate's degree in Nursing; BSN preferred
  • 2-4 years of RN experience in hospice care, regulations, management, administration, marketing, education, or discharge planning
  • Ability to collaborate with others and initiate and plan own work activities
  • Effective patient and staff teaching
  • Maryland Registered Nurse License, American Heart Association CPR Healthcare Provider Certification
