Job Summary
As a Nurse Specialist with LifeBridge Health, you will provide professional, comprehensive, family-oriented nursing care to individuals in the home setting during the final stages of life. Your expertise will be essential in assessing patients' and families' needs and formulating, implementing, and evaluating a plan of care. You will perform primary care case management functions for assigned patients, ensuring that all services are delivered in accordance with the Mission Statement and S.P.I.R.I.T. Values of Carroll Hospice and Carroll Hospital Center.
Responsibilities
Assess patients' and families' needs and develop individualized plans of care
Provide professional nursing care to patients in the home setting
Collaborate with interdisciplinary teams to ensure comprehensive care
Perform primary care case management functions for assigned patients
Requirements
Associate's Degree in Nursing from an accredited school
Minimum of two years of recent medical/surgical nursing experience
One year of home health experience preferred
Expertise in performing health assessments
Certification in Hospice and Palliative Nursing (CHPN) preferred
