Weeks Shift:

Day Shift, 8:00am - 4:30pm Salary: $71:00 per hour Responsibilities: Conduct comprehensive assessments to determine the healthcare needs of patients.

Develop and implement individualized case management plans to ensure optimal patient care. Collaborate with healthcare professionals, social workers, and other stakeholders to coordinate and facilitate patient care. Monitor and evaluate patient progress and adjust care plans as needed. Advocate for patients to ensure access to appropriate resources and services. Maintain accurate and detailed documentation of case management activities.

Requirements:
Registered Nurse (RN) licensure. Experience in case management or care coordination. Knowledge of healthcare resources, insurance, and reimbursement processes. Basic Life Support (BLS) certification.
