Job Title: AHD Case Manager RN

Alameda Health System is seeking a skilled and compassionate AHD Case Manager RN to join our team. As a key member of our healthcare team, you will play a vital role in providing comprehensive case management services to clients with complex health conditions and social challenges.

Job Summary:

The AHD Case Manager RN is responsible for coordinating care for patients with complex health needs, ensuring timely and effective delivery of services to prevent health status decline and reduce the need for costly services. This role requires strong communication and interpersonal skills, as well as the ability to work effectively with patients, families, and healthcare teams.

Key Responsibilities:
  • Conduct comprehensive assessments of patients' needs and develop individualized care plans to address their health and social challenges.
  • Coordinate with healthcare teams to ensure timely and effective delivery of services, including hospitalization, home health care, and community resources.
  • Provide education and support to patients and families to promote self-management and independence.
  • Collaborate with payers and other stakeholders to secure necessary services and benefits.
  • Monitor patients' progress and adjust care plans as needed to ensure optimal health outcomes.
Requirements:
  • Bachelor's degree in a related field (Master's degree preferred).
  • Valid license to practice as a Registered Nurse in the State of California.
  • Basic Life Support Certification issued by AHA-American Heart Association.
  • Varied clinical experience or experience in case management/community health preferred.
