Job Title: RN Case Manager

We are seeking a skilled RN Case Manager to join our team in Oakland, CA.

About the Role:

The successful candidate will be responsible for managing the care of patients through the utilization review process, ensuring that patients receive the necessary care and services to achieve optimal health outcomes.

Key Responsibilities:
  • Conduct thorough assessments of patients' medical needs and develop individualized care plans.
  • Collaborate with healthcare providers, payers, and other stakeholders to ensure seamless care coordination.
  • Monitor patients' progress and make adjustments to care plans as needed.
  • Ensure compliance with regulatory requirements and industry standards.
Requirements:
  • 2 years of relevant experience in utilization review or case management.
  • Must have experience with Careport/All Scripts and other relevant software systems.
  • Must be available to work 2/3 of major holidays.
  • Must have experience with admission criteria, benefits eligibility, care coordination, disability care management, discharge planning, needs assessment, and other relevant areas.
