Job Summary

We are seeking a skilled and compassionate Registered Nurse to join our team as a Connected Care Coordinator. In this role, you will serve as a key patient advocate and educator, coordinating post-acute care services within our AdventHealth network.

Key Responsibilities
  • Conduct thorough assessments of patients to determine appropriateness for post-acute care settings
  • Inform and educate patients and families about post-acute care options, balancing their requests with safe and reliable care
  • Identify patient and family problems or needs, ensuring communication with physicians, care management, and the clinical team
  • Coordinate home health care referrals within assigned hospitals, conducting bedside assessments to determine appropriateness of home or hospice care admission
Requirements
  • Associate in Nursing or above
  • Minimum two years of post-acute experience or Care Management in a facility
  • Experience working with the public and exceptional customer service skills
  • Knowledge of medical terminology and the post-acute referral process
  • Registered Nurse
  • Basic Life Support certification
  • Valid Colorado driver's license, automobile insurance, safe driving record, and reliable transportation
