NYC Health + Hospitals is seeking a highly skilled and experienced CDTM Pharmacist to join our team. As a CDTM Pharmacist, you will play a critical role in providing exceptional patient care and optimizing medication therapy in the ambulatory care setting.
Job Summary:The CDTM Pharmacist will work collaboratively with the Office of Ambulatory Care and collaborating physicians to support the mission of providing exceptional patient care across the Health System. You will serve as a liaison between the patient, ambulatory, clinical, and pharmacy teams, leading to efficient provision of drug therapy and positive patient outcomes.
Key Responsibilities:- Interview patients to gather relevant data, including medical history, medication history, and immunization history.
- Screen for drug allergy and drug interactions, and note contraindicated drugs or classes of drugs.
- Assess medication regimens and make modifications to achieve desired therapeutic outcomes.
- Perform routine patient monitoring functions, including collecting and reviewing patient history, and ordering laboratory and physical assessments as necessary.
- Adjust and/or manage drug regimens in accordance with patient-specific written orders or protocols.
- Document modifications made to patient drug therapy and notify other treating providers as required.
- Communicate changes or adjustments in therapy to patients and caregivers in a clear and concise manner.
- Monitor patient response to therapy, including safety and effectiveness of medication.
- Present requested drug information to patients, physicians, nurses, and other pharmacists.
- Provide patient education on self-management and assist patients and providers with medication access issues.
- A valid New York State license and current registration to practice pharmacy.
- Certification to practice Collaborative Drug Therapy Management (CDTM) by the New York State Education Department (NYSED).
- Proficiency with Microsoft Visio, Project, Excel, Word, and standard computer programs.
- Prior experience with EPIC EMR/Willow Ambulatory.
- Advanced interpersonal skills and excellent oral and written communication skills.
- Immunization Certification and Current BLS Certification.
