Key Responsibilities- Provide comprehensive medication reviews and disease management services to patients via telephone or telehealth technologies.
- Assess medication therapies and identify medication-related issues and disease care gaps.
- Monitor drug therapy regimens for contraindications, drug-drug interactions, and appropriateness of drug and dose.
- Suggest cost-effective therapeutic medication alternatives to medical staff.
- Communicate effectively with internal team members, community partners, and stakeholders to plan and execute clinical activities.
- Maintain proper documentation of clinical activities in a proprietary software application.
- Provide education and training on medication use and monitoring devices.
- Collaborate with an interdisciplinary team to provide pharmacy and medication-related client activities.
- Remain current with new initiatives, activities, and techniques through research and continuous education.
- Graduate from an American Council of Pharmaceutical Education accredited College of Pharmacy.
- Licensure or eligibility for licensure.
- Board certification through the BPS (BCGP, BCPS, BCACP) or willingness to become certified within 12 months.
- Excellent interpersonal and communication skills.
- Ability to manage multiple priorities and work independently.
- Proficient with Google Office Suite and Windows.
