Job Title: RN Coordinator

The RN Coordinator plays a vital role in ensuring the seamless transition of patient care from hospital discharge to follow-up appointments. This position requires strong organizational skills, critical thinking, and attention to detail to coordinate patient care, procedures, and hospital admissions.

Key Responsibilities:
  • Facilitate patient plan of care, as outlined by the provider during office visits.
  • Review patient Health Risk Assessments to identify potential barriers, risk factors, opportunities for education, and gaps in recommended preventive services.
  • Provide education and instructions for procedures and/or testing ordered by providers.
  • Coordinate patient procedures and possible hospital admissions, requiring coordination with both inpatient and outpatient services.
  • Participate in patient clinical appointments, assisting in patient care.
  • Review and complete FMLA & disability paperwork for patients and caregivers.
  • Prepare patients for visits by gathering baseline data, documenting reasons for the visit, and obtaining vitals/office testing.
  • Perform specific specialized tests, such as pulmonary function tests and phlebotomy procedures, as needed.
  • Anticipate and prepare patients, clinical equipment, and supplies for visits.
  • Assist and/or perform procedures alongside physicians.
  • Triage patient calls, gathering and reviewing eligible patient self-reported health information.
  • Coordinate care for patients with providers and/or other service lines/specialties.
  • Furnish personalized health advice to patients and provide referrals as appropriate for recommended testing or preventive counseling.
  • Engage patients in self-management.
  • Monitor patient charts for test results, triage results, and review for abnormalities with ordering providers.
  • Communicate test results to patients in a timely manner and facilitate any further orders outlined by providers.
  • Review and manage preoperative surgical clearance requests with providers, if needed.
Requirements:
  • Graduation from an accredited Nursing Program.
  • Active Registered Nurse License in the state of assigned practices.
  • ACLS and BLS certification.
  • EPIC experience preferred.
  • 2+ years of relatable experience preferred.
Working Conditions:

Frequent sitting and computer use.
