Overview

AccentCare is seeking a dedicated and compassionate Home Health LVN/LPN to join our team. As a medical professional, you will have the opportunity to make a meaningful impact on the lives of our patients and their families.

Responsibilities
  • Provide high-quality nursing care to patients in their homes, following treatment plans prescribed by physicians.
  • Conduct thorough assessments of patients' medical needs and develop individualized care plans.
  • Collaborate with healthcare teams to ensure seamless care coordination and communication.
  • Maintain accurate and complete medical records, adhering to regulatory requirements.
Requirements
  • One year of clinical experience and one year of home health experience.
  • Ability to provide nursing visits according to treatment plans and maintain accurate medical records.
  • Graduate of an accredited practical or vocational nursing program.
  • Currently licensed practical nurse or licensed vocational nurse in the state of agency operation.
  • Valid CPR certification.
  • Licensed driver with ability to travel to all business locations.
