We're seeking a highly skilled Patient Care Navigator RN to join our team. As a Patient Care Navigator RN, you will play a critical role in ensuring patients receive the highest quality care during their hospital stay. You will work closely with our interdisciplinary team to assess patients' needs, develop discharge plans, and identify barriers to safe discharge.

This is a full-time position working weekends. The ideal candidate will have excellent communication and problem-solving skills, as well as the ability to work effectively in a fast-paced environment. About the Role
The Patient Care Navigator RN is responsible for assessing, developing, planning, implementing, and evaluating patients' discharge planning needs. This includes identifying potential barriers to safe discharge and collaborating with the interdisciplinary team to develop solutions. The successful candidate will have excellent communication and problem-solving skills, as well as the ability to work effectively in a fast-paced environment. We estimate the annual salary for this position to be around $83,000 - $110,000 based on national averages for a registered nurse with similar experience and qualifications. This estimate may vary based on individual circumstances and does not include additional forms of compensation, such as bonuses or overtime pay.
