Patient Navigation Coordinator Job Description

Seeking a Patient Navigation Coordinator to assess patients' barriers to care and make appropriate referrals to remove barriers to effective breast health care services.

  • Assess patients' barriers to care and make referrals to remove barriers to effective breast health care services.
  • Advocate for and support patients and caregivers through our team and support services, providing equitable care and outcomes.
  • Recruit and match patients for the Breast Cancer Peer Support Advocacy Program.
  • Promote the Breast Center program through community outreach activities with local groups and organizations.

Requirements
  • A minimum of a Bachelor's Degree in social work, counseling, nursing, or a related field.
  • One year of experience working with underserved patient populations.
  • Preferred: Master's Degree or RN, LPN, or Social Worker with oncology experience.
