Job Summary

The Advanced Practice Fellow at Vanderbilt University Medical Center is a key member of an interdisciplinary healthcare team, working collaboratively to deliver high-quality patient care. This role requires a strong foundation in evidence-based practice, with a focus on developing and refining essential clinical and critical thinking skills.

Key Responsibilities
  • Assess patient health status through comprehensive history and physical examinations, documenting findings in the medical record.
  • Order and collect relevant diagnostic data, using appropriate assessment techniques and supporting information.
  • Participate in daily huddles or rounds with the interdisciplinary team, actively collaborating in formulating and evaluating ongoing plans of care.
  • Collaborate with the healthcare team to make accurate diagnoses of acute and complex chronic conditions, managing diagnostic tests and formulating differential diagnoses.
  • Develop and implement individualized plans of care, utilizing evidence-based practice guidelines and protocols.
  • Prescribe diagnostic strategies and therapeutic interventions to achieve expected outcomes, managing further diagnostic tests and performing advanced procedures as needed.
  • Maintain ongoing communication and collaboration with the interdisciplinary healthcare team, facilitating communication with patients, families, and staff to promote continuity of care.
  • Document key components of patient progress, ensuring timely and compliant documentation that captures patient acuity and complexity.
Requirements
  • Master's degree in a relevant field.
  • Less than 1 year of relevant work experience.
  • Certifications as required by the position.
