Job Summary

The Vanderbilt University Medical Center is seeking an Advanced Practice Fellow to join our interdisciplinary healthcare team. As a key member of our team, you will work collaboratively with healthcare professionals to provide high-quality patient care.

Key Responsibilities
  • Assessment of Health Status
    • Conduct thorough patient assessments, including history and physical examinations.
    • Order and collect relevant diagnostic information and procedures.
    • Identify potential risks associated with care, including medication side effects and fluid and electrolyte imbalance.
  • Diagnosis
    • Collaborate with the healthcare team to make accurate diagnoses of acute and complex chronic conditions.
    • Manage diagnostic tests, including ordering, interpretation, and performance.
    • Formulate differential diagnoses and identify appropriate interventions.
  • Formulates Plan of Care
    • Identify expected outcomes from diagnosis and develop a plan of care to address acute and complex health care needs.
    • Utilize evidence-based practice guidelines and protocols to inform individualized care plans.
  • Implements and modifies plan of care
    • Prescribe diagnostic strategies and therapeutic interventions to achieve expected outcomes.
    • Manage further diagnostic tests and procedures.
    • Perform advanced procedures consistent with privileges and competency validation.
  • Communication and Collaboration
    • Maintain ongoing communication with the healthcare team and facilitate communication with patients, families, and staff.
  • Documentation
    • Document key components of patient progress, including daily progress notes, transfer, and discharge summaries.
  • Professional Practice
    • Provide preceptorship, education, and instruction to students and nursing staff.
    • Engage in research and analysis of evidence-based practice.
    • Attend educational opportunities, including Grand Rounds, AP Council meetings, and faculty meetings.

  • Certifications: As required by state guidelines
  • Work Experience: Less than 1 year
  • Education: Master's
