Job Role: Child Welfare Coordinator - Foster Care
Objective: To deliver comprehensive case management services for children entering or currently in out-of-home care, ensuring effective placement, stabilization, and access to necessary services for these children and their families, with a strong emphasis on safety and permanency.
This role embodies a commitment to the mission, vision, and values of Positive Impacts by fostering respectful and collaborative relationships with clients, team members, and external partners. The coordinator is accountable for providing essential services while empowering clients to achieve the organization's goals.
Key Responsibilities:
- Make timely decisions regarding client services, ensuring the provision of medical, educational, social, and emotional support.
- Provide emergency availability to clients and families through a dedicated contact system, 24/7.
- Develop and implement permanency goals and plans for assigned cases.
- Maintain thorough documentation of all client interactions, including assessments, treatment plans, and case evaluations.
- Conduct bi-monthly meetings with biological parents, children, and caregivers in compliance with licensing and accreditation standards.
- Facilitate Family Support Team (FST) and Team Decision Making (TDM) meetings as necessary.
- Coordinate visits between parents, children, and siblings as per contractual obligations.
- Identify and connect clients with community resources to enhance service delivery.
- Monitor client progress towards outcomes and document achievements to demonstrate service effectiveness.
- Collaborate with other child service staff, legal entities, and out-of-home care providers.
- Attend court hearings and prepare necessary documentation in accordance with established policies.
- Transport clients to appointments, meetings, and other necessary locations.
- Ensure legal permanency for each case managed.
- Participate in Peer Record Reviews for quality assurance.
- Adhere to all agency policies, including attendance, safety protocols, and confidentiality standards.
Qualifications:
A Bachelor's degree in social work, counseling, psychology, sociology, or a related field is required. A minimum of one year of professional experience in protective social services or case management is preferred. Demonstrated ability to work effectively within a team and engage with community resources is essential.
Language Proficiency: Candidates must possess the ability to read and interpret business documents and write clear reports and correspondence. Strong presentation skills are necessary for effective communication with various stakeholders.
Reasoning Skills: Candidates should be able to identify problems, gather relevant data, and draw logical conclusions. Independent decision-making and problem-solving abilities are crucial for success in this role.
Physical Requirements: The role involves regular communication and may require sitting, standing, walking, and occasional lifting of up to 25 lbs.
Work Environment: Most duties will be performed in community settings, with responsibilities including client transportation.
Additional Requirements: Candidates must be at least 21 years old, possess a valid driver's license, and have reliable access to a vehicle with appropriate insurance.
