**Job Summary**
We are seeking a skilled Licensed Practical Nurse (LPN) to join our care team. The successful candidate will assist residents with daily care, provide nursing functions as directed, and maintain resident confidentiality. **Job Responsibilities**
- Assist residents with daily bath, dressing, grooming, dental care, bowel and bladder functions, preparation for medical tests and exams, ear and eye care, and positioning in and out of beds, chairs, bathtubs, etc.
- Provide nursing functions as directed by supervisor, including daily perineal care, catheter care, turn residents in bed, all bathing, measure and record temperature, pulse, and respiration, weigh and measure residents, perform restorative and rehabilitative procedures, and observe and report the presence of skin breakdowns.
- Prepare residents for meals and snacks, identify food arrangements, and assist in feeding residents as needed; record food and fluid intake and perform after meal resident care as needed.
- Maintain resident confidentiality, treat residents with kindness, dignity, and respect, know and comply with Resident's Rights rules, and promptly report all resident complaints, accidents, and incidents to supervisor.
- Utilize nursing knowledge and skills in the safe implementation of basic preventive, therapeutic, and rehabilitative nursing care of assigned patients/families as evidenced by documentation and observation of positive patient care outcomes.
- Perform delegated independent nursing functions using established procedures, policies, guidelines, and standards as observed by the registered nurse.
- Assist in data collection on the Admission Data Record.
- Contribute to the development of the patient/significant other plan of care based on the identified patient problems/needs.
- Carry out physician orders accurately, directly, and indirectly.
- Assist in the evaluation of the effectiveness of the nursing care administered relative to the patient/significant other identified problems/needs.
- Document pertinent observations and interventions on appropriate records.
- Perform selected tasks related to patient care and the patient's environment in a safe manner.
- Administer medication accurately, observing patient response, as evidenced by documentation in the medical record and lack of negative outcomes.
- Perform limited IV therapy procedures on those patients receiving peripherally inserted intravenous fluids, as delegated by and observed by the RN.
- Maintain and coordinate communication among patient/significant other, staff, and physician as evidenced within medical records, client, and healthcare team feedback.
- Give punctual change of shift report consisting of pertinent and relevant information for assigned patients.
- Provide patient/significant other health teaching as delegated by and observed by the RN.
- Use available written and AV materials specific to patient condition or disease.
- Utilize appropriate teaching methods such as one-on-one and group instruction, demonstration, or discussion.
- Participate in staff development programs and meetings as requested.
**Required Skills and Qualifications**
- Graduate of a nationally approved Nursing program is required.
- Maintain current BLS/CPR certification from either the American Heart Association (AHA) or the American Red Cross (ARC).
- Must be currently licensed by the State Board of Nursing.
