Job Summary
">- We are seeking a skilled Physical Therapist to join our team at our Assisted Livings facilities.
About the Role
">- The Physical Therapist will evaluate and treat patients in accordance with physician orders, utilizing therapeutic procedures to address physical function deficits and improve independence, safety, and quality of life.
Responsibilities
">- Evaluate patient information and assess its impact on treatment.
- Conduct thorough functional assessments using standardized procedures, including home and vocational needs.
- Develop and implement individualized treatment plans based on patient needs and containing achievable functional goals.
- Comply with evaluation, treatment, and documentation guidelines.
- Record patient progress every seven days.
- Revise treatment plans as needed throughout the treatment process.
- Document treatment course clearly and concisely, including discharge summaries.
- Instruct assistants in treatment procedures and provide close supervision.
- Orient patients to program services and treatment procedures.
- Work with nursing staff, patients, and families to maximize functional abilities.
- Establish and instruct patients and caregivers in exercises and therapeutic procedures.
- Assist in referral processes when patients require additional services after discharge.
- Attend rehabilitation team conferences regarding patient progress, problems, or needs.
- Participate in quality improvement activities by responding to medical record audits, patient/referral source satisfaction surveys, safety activities, and regulatory compliance.
- Act in compliance with management policies, State, Federal, and Professional Organization regulatory standards and guidelines.
- Promote Vertis Therapy programs and services through interactions with the community.
Requirements
">- Master's degree in Physical Therapy or Bachelor's degree in Physical Therapy with five years of direct practice experience.
- Current license/certification in the state of practice.
- Prefer one year of experience in rehabilitation, supervision, training, or experience in a setting serving the same age/type of patient population served by this facility.
- Prefer membership in the American Physical Therapy Association.
- Demonstrated competency in identified areas of specialization.
- Excellent oral and written communication and interpersonal skills.
- Adequate computer skills for job duties and documentation.
- Demonstrated competency in applying principles, methods, materials, and equipment used in physical therapy.
- Knowledge of accreditation standards and compliance requirements.
Compensation
">- The estimated salary for this position is $90,000 - $110,000 per year, depending on experience.
