About the Role:
In this rewarding position as a Healthcare Navigator, you will play a vital role in coordinating care for high-risk health plan members residing in Senior Community Settings. As part of an interdisciplinary team, you will ensure accurate and complete documentation, communicate effectively with care teams, and assist providers with various care coordination activities.

Key Responsibilities:
- Ensure accurate and complete documentation
- Communicate and collaborate with the interdisciplinary care team
- Assist providers with benefit determination associated with Medicare Part A, B, and D benefits
- Collaborate with primary advanced practice clinicians, care navigators, and responsible parties to identify and respond to patient changes in condition

About You:
- Associate's degree or higher in Nursing (RN)
- Active and unrestricted license in the state where you reside or ability to obtain by start date
- Compact licensure or willingness to obtain within 90 days of start date
- Certified in Basic Life Support
- Ability to complete physical requirements, including lifting a 30-pound bag and performing physical assessments

What We Offer:
- An estimated salary of $65,000 - $85,000 per year based on experience and location
- Flexible scheduling with no on-call work required
- Opportunity to achieve work-life balance with 24 hours of minimum work per month and a maximum of up to 80 hours per month
